Community First Choice

Maryland’s Community First Choice option provides community services and supports to enable older adults and people with disabilities to live in their own homes.

Eligibility Guidelines

Medical and Technical Criteria

  • Individuals must reside in the community.
  • Individuals must meet the level of care 
    required to qualify for services in an institution.

Financial Criteria

  • Individuals must qualify for Medicaid in the community
  • Individuals can apply for Medicaid at anytime
  • Individuals who receive Supplemental Security Income (SSI) are automatically
    eligible for Medicaid and do not need to apply
  • Medicaid coverage information can be found at http://mmcp.health.maryland.gov
